Must be printed, filled out by physician and uploaded at the time of registration.
Students will NOT be authorized to participate in practice or tryouts until submitted for review.
Form needs to be filled out by EVERY athlete every three (3) years.
Well check-up paperwork will NOT be accepted.
Done during online registration.
Must be filled out once a year in order to participate in a high school sport or activity.

Parent Permission to Transport Student Form 📄
Only the parent/guardian is allowed to transport their student.
The coach must be notified 24 hours in advance that the student will not be taking the bus.
